Piranhas aggressively swarm anything entering the water, so avoid rivers when possible and keep tames away from shorelines or passive water pens. If you must fight, lure them into shallow water or onto land and attack from safety with ranged weapons or a strong melee weapon; a Sarco can also scare them away.
